HFEA inspection of UK fertility clinics covers the full range of clinical, scientific, governance and premises requirements set out in the HFEA Code of Practice, 9th edition. For the laboratory specifically, several areas receive consistent attention from inspectors.

Sample Identification and Traceability

Sample identification and traceability is one of the most scrutinised areas. HFEA requires that every sample in storage can be positively identified at any point. Inspectors assess the physical identification system – straws, visotubes, goblets, canisters, vessel inventory records – and check that the physical contents of storage vessels match the inventory records. Discrepancies are a finding.

Equipment Maintenance Records

Equipment maintenance records are assessed during inspection. For liquid nitrogen storage vessels, this means checking that consumption logs are being kept and that backup storage is in place and charged. For controlled rate freezers, calibration certificates and profile validation records should be available.

Witness Protocols and Consent

Witness protocols are reviewed for both policy adequacy and evidence of consistent application. Consent documentation for stored samples, including evidence of consent renewal for samples approaching ten-year storage periods, is reviewed in the context of the Health and Care Act 2022 requirements.

HFEA inspection does not announce every finding in real time. Equipment maintenance gaps and traceability failures are among the most common routes to conditions – requirements that must be addressed before the licence can be maintained.
